A. Types of Information We Collect

1.  Personal Information

This is information that is associated with the Registrar's name or personal identity.  This necessary information is used in record keeping to track participation in activities conducted under the auspices of the training program and to build a data base of qualifications and credentials achieved during the period with which the Registrar is associated with the regional training provider.

When requested appropriate information relating to participation, qualifications, etc will be provided to GPET for its records.

Personal contact addresses will not be divulged without the express written approval of any individual involved with the training program.

2.  Information Relating to Training Practices and Supervisors

As a normal part of the ongoing operations of the training provider, data will be received from individual practices.  This information is used to reimburse practices and/or supervisors for services provided to the training program.

This data will be used in the ongoing review and quality improvement process.  Data relating to individual practices will not be divulged to any other practice or individual.  Data from across the region will be aggregated according to need.  Individual practices will be able to compare their own performance against that of the aggregated data for the consortia.

3.  Survey Data

From time to time individuals and/or training practices will be surveyed on different aspects of the training program as part of its ongoing quality improvement program.  Where possible, such surveys will be designed so that the respondents will not be identified.  All data will then be aggregated for benchmarking and comparative purposes.

When it is not possible to have de-identified data, this will be used only for internal administrative purposes and this information will not be divulged to any other third party, unless prior approval is given by the Registrar.  Only aggregated data would be used.
